Please remember to take the physical form (with the medical history portion completed and signed), a list of your child’s medications, and the Medication Authorization Form with you to the appointment.

If your child’s immunizations are not recorded on the attached physical form, please take all immunization records with you for the physical examination.

REMINDER:

The required physical and immunizations are due at August registration dates. You may turn them into the school early to avoid losing them if you wish.

**Please note that the physical form required for the school can be used as a sports physical. However, the IESA/IHSA physical forms cannot be used in place of a school physical.

They have clinics in addition to the Dental Bus that travels.

Charleston Health Services- Now provides dental services to children under the age of 21 who are covered by Medicaid. They are located at 626 W. Lincoln Ave, Charleston IL. The phone number is 217-345-7702.
